Mchenry County, IL. – El Departamento de Salud del Condado de McHenry (MCDH, por sus siglas en inglés) investiga cualquier enfermedad que haya ocurrido a partir del 7 de febrero y tiene una asociación con la escuela Crystal Lake Montessori, 3013 S. Country Club Rd. en Woodstock, o en cualquier evento en un lugar alternativo patrocinado por la escuela.
La investigación está en curso y, por lo tanto, la información sobre el número total de casos de enfermedad y la causa de la enfermedad no está disponible actualmente.
MCDH solicita que todas las personas que comieron alimentos en un evento en el aula de la escuela Crystal Lake Montessori o en un evento patrocinado por la escuela, independientemente de si desarrollaron síntomas, completen una encuesta confidencial disponible en redcap.link/foodillnesssurvey.
MCDH también solicita que cualquier persona que esté asociada con la escuela y se enfermó pero no asistió a ninguno de los eventos patrocinados por la escuela el 7 de febrero o después, también complete esta encuesta, ya que ayudará en la investigación general.

MCDH investigating illness by local school
The McHenry County Department of Health (MCDH) is investigating any illnesses that occurred on or after Feb. 7 and have an association with Crystal Lake Montessori School, 3013 S. Country Club Rd. in Woodstock, or at any events at an alternative location sponsored by the school.
The investigation is ongoing and therefore information around the total number of cases of illness and the cause of illness is currently not available.
MCDH is requesting that everyone who ate food either at a Crystal Lake Montessori School classroom event or from a school-sponsored event, regardless if they developed symptoms, complete a confidential survey available at redcap.link/foodillnesssurvey. MCDH is also asking that anyone who is associated with the school and became ill but did not attend any of the school-sponsored events on or after Feb. 7 also complete this survey as it will aid in the overall investigation.
Parents who complete the survey on behalf of their child, please answer the questions respective to their experience. Information collected in the survey is confidential. In addition, the survey database used to collect this information is HIPAA compliant. The survey will only take a few minutes to complete and is important to help understand what happened to cause illnesses.
