Waukegan, IL. – La secretaria del condado de Lake, Robin M. O’Connor, emitió un recordatorio para que los votantes registrados que ingresan en un hospital, hogar de ancianos o centro de rehabilitación dentro de los 14 días anteriores a la elección primaria general del 28 de junio de 2022 pueden calificar para la entrega personal de una boleta por un familiar o amigo, sujeto a las condiciones legales.
Los votantes elegibles deben completar el formulario de solicitud titulado Solicitud de boleta para votante calificado admitido en un hospital, hogar de ancianos o centro de rehabilitación y su médico debe completar la Declaración jurada del médico tratante adjunta.
Todos los formularios requeridos deben ser llenados y devueltos a la oficina del Secretario del Condado de Lake antes de que se emita el Certificado de Solicitud y la boleta. Los formularios están disponibles comunicándose con el Departamento de Elecciones del Secretario del Condado de Lake en VotingByMail@lakecountyil.gov o 847-377-VOTE. Voters can vote while hospitalized
Lake County Clerk Robin M. O’Connor issued a reminder today that registered voters who are admitted to a hospital, nursing home, or rehabilitation center within 14 days before the June 28, 2022, General Primary Election may qualify for personal delivery of a ballot by a family member or friend, subject to statutory conditions.
Eligible voters must complete the request form titled Application for Ballot for Qualified Voter Admitted to a Hospital, Nursing Home, or Rehabilitation Center and their physician is required to complete the accompanying Affidavit of Attending Physician.
All required forms must be completed and returned to the Lake County Clerk’s office prior to the Certificate of Application and ballot will be issued. Forms are available by contacting the Lake County Clerk’s Election Department at VotingByMail@lakecountyil.gov or 847-377-VOTE. Or, registered voters may download the Hospitalized Voter Form by visiting LakeCountyClerk.info, clicking on the “June 2022 Primary” icon, and following the link to “Hospitalized Voter.”
