District 60 Health Requirements for school

Waukegan, IL. -The Illinois Department of Public Health requires a number of immunizations and exams before students can attend school in the State of Illinois. Below is a detailed list of those requirements and the grades affected by each prerequisite. Parents are asked to please begin planning for the 2018/2019 school year and begin scheduling these immunizations soon.
All students entering Preschool, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Health form. A current physical is one that was completed within one year of August 23, 2018. A sport physical does not meet this requirement. The parent portion on the backside of the physical form must be completed and signed.

Immunization requirements
All students must be current with all Illinois immunization requirements including the following:
All students entering kindergarten, first, second, third, fourth and grades six through twelve must show proof of receiving two doses of Varicella (chickenpox) vaccine.
All students entering sixth, seventh, eighth and ninth grade must show proof of receiving one does of the Meningococcal (meningitis) vaccine.
All students entering twelfth grade must show proof of receiving two doses of Meningococcal (meningitis) vaccine unless the first dose was given after the age of 16, then only one dose is required.
All students entering grades six through twelve must show proof of receiving one dose of Tdap vaccine.

Dental Examination Requirement
Illinois Dental examinations forms are due by May 15th of the year the student is enrolled in kindergarten, second and sixth grade. A current examination is one that was completed within 18 months for May 15th.

Eye Examination Requirement
Illinois Eye examination reports are due by October 15th for students enrolling in kindergarten or for the first time in an Illinois public school. A current examination is within year of enrollment.

District Awarded in Financial Reporting
Waukegan Community Unit School District # 60 been again been awarded a Certificate of Excellence in Financial Reporting from the Association of School Business Officials International (ASBO).
District 60 and its Finance Department has been recognized by ASBO for its Comprehensive Annual Financial Report (CAFR) for the fiscal year ending 2017. This is the 11th consecutive year that ASBO has recognized District 60 with a Certificate of Excellence. A Certificate of Excellence is awarded to districts that met or exceeded the program’s high standards for financial reporting and accountability.
The Certificate of Excellence in Financial Reporting award was created by ASBO to confirm the school business office’s commitment to financial accountability and transparency. Applicants submit their CAFR for review by a team of professional auditors, who provide feedback to improve future documents. If the CAFR meets the requirements of the program, the document may receive the Certificate of Excellence. Recognition of the program can help strengthen a district’s presentation for bond issuance statements.
